Consent: WHAT IS ACHA CONSENT FORM?

Hope:  Can you please explain IEP “Consent for Release of Information to the Agency for Health Care Administration”?  I keep asking but never get a real answer as to what we are signing. Thank you

  1. ACHA = Agency for Health Care Administration They are involved with administering the federal HIPPA law, that deals with sharing medical info. The school should know what info they will be sharing & with whom.
